Intraperitoneal and Subsequent Intravenous Vancomycin: An Effective Treatment Option for Gram-Positive Peritonitis in Peritoneal Dialysis

ثبت نشده
چکیده

Methods: In April 1996, we initiated a protocol for treating peritonitis caused by Gram-positive organisms using a 2-g intraperitoneal loading dose of vancomycin followed by intravenous vancomycin at 1 g twice in 5 days for coagulase-negative Staphylococcus and at 1 g three times in 5 days for Staphylococcus aureus. We analyzed episodes of Gram-positive peritonitis (coagulase-negative and S. aureus) and the effi ciency of the treatment protocol in 113 patients undergoing peritoneal dialysis between 1 April, 1996 and 3 August, 2016. There were 6090 patient-months and the mean treatment lasted 54±44 months. The outcomes were evaluated as (1) complete cure, (2) relapsing peritonitis, (3) catheter removal for refractory peritonitis, and (4) death.

Immunoglobulin E-mediated hypersensitivity reaction after intraperitoneal administration of vancomycin

Intraperitoneal (IP) vancomycin is widely used to treat Gram-positive peritonitis associated with peritoneal dialysis. There have been two cases of red man syndrome (RMS), a vancomycin-specific nonimmunologic reaction, associated with IP vancomycin. However, immune-mediated hypersensitivity reaction to IP vancomycin has not yet been reported. A 49 year old woman on continuous ambulatory periton...

Cefepime versus vancomycin plus netilmicin therapy for continuous ambulatory peritoneal dialysis-associated peritonitis.

Cefepime is a cephalosporin with a broad spectrum of activity against most gram-positive and gram-negative pathogens. In this study, we attempted to compare the safety and efficacy of cefepime monotherapy against the potentially more toxic combination of vancomycin and netilmicin in the treatment of continuous ambulatory peritoneal dialysis (CAPD)-associated bacterial peritonitis. Eighty-one co...

Vancomycin therapy for treatment of peritonitis in outpatients on peritoneal dialysis.

Vancomycin therapy is frequently used for peritonitis in patients on peritoneal dialysis, but the emergence of resistance has been reported. We evaluated the efficacy of a single intraperitoneal dose of vancomycin for peritonitis in peritoneal dialysis patients. We assessed 24 episodes of peritonitis in 16 patients, and compared clinical parameters between responders and nonresponders. Vancomyc...

Analysis of peritoneal dialysis-related peritonitis pathogenic bacteria and its drug-resistance

Objective: Understand the pathogen spectrum changes of patients with peritoneal dialysis-related peritonitis, and provide a theoretical basis for the prevention of peritonitis, guided by a rational use of medicines in clinical treatment. Methods: Perform a retrospective analysis of the pathogen spectrum, drug sensitivity test results and disease prognosis from 76 cases, admitted to hospital in ...

Coagulase-negative staphylococcal peritonitis: outcomes of cephalosporin-resistant strains.

Peritoneal dialysis (PD) peritonitis and subsequent relapses are undesirable complications for patients requiring home peritoneal dialysis. Coagulase-negative staphylococci (CoNS) remain a common cause of peritonitis. Strains of CoNS are emerging that are resistant to cephalosporins. It has been suggested that, if sensitivity testing shows resistance to cephalosporin but the patient is improvin...
